New patients should plan for a two-hour experience at our office. The time includes the consultation, all testing, and meeting with the surgery coordinator.

Schedule an appointment with us.
After your appointment is scheduled, you will receive a text or email to register for our patient portal.
Fill out the required online registration forms found on our patient portal before your appointment. This will help streamline your appointment.
With your online registration forms complete, arrive 5-10 minutes before your appointment.
Arrive 15 minutes prior to your appointment if you DID NOT complete our online registration forms. You will need to fill them out in-office.
We are not able to perform same day surgeries. If it is an emergency, please call 911.
You will receive an email or text message to the patient portal once an appointment has been scheduled. Follow the steps in the email or text message to gain access. If you cannot find your invite to register, please call us. We can resend you an invite to your email on file.
If you are enrolled in a managed care health plan (HMO), you will need a referral or authorization from your Primary Care Physician (PCP) prior to your appointment in our office. If authorization is not obtained, you will be responsible for the bill.
